For me, its not necessarily always defined moments, but trends:

1.) The poorly executed all-stars season
2.) Expansion beyond 16 castaways/2 tribes and dividing the tribes up by age/race/gender/etc. It made sense in the Amazon and Vanuatu when there was a men vs women aspect to the culture, but not with Panama and Cook Islands.
3a.) Focusing 80% of the show on the challenges and tribal councils (really seemed over-the-top noticeable around S12 or S13) and not developing all the castaways and their stories.
3b.) Probst taking over the show by narrating too much and talking through challenges
4.) Recycling locations
5.) Shoving certain characters down our throats (Rupert, Stephenie, Terry, James, and now Coach)
6.) Gimmick castaways like a former-NFL quarterback, Astronaut, Fighter Pilot, WWE Diva, 90's music star, etc.....and made up castaways/cliche castaways like "Model", "Fitness Trainer" and Mechanical Bull Operator
7.) Hidden Immunity Idols and Tribe Switches ruining the game for otherwise well-positioned players, just for the sake of unpredictability......yet editing in such a way that it isnt hard for regular viewers to easily figure out who the major players will be in the game.
