1) Bob crafts a realistic fake idol - He may be creative and handy, but in a show where you don't even have spare underwear, how are you going to construct a fake idol?
2) Bob tells Sugar about the fake idol. - Up until that point, Bob and Sugar barely ever spoke. If he's going to confess having a fake idol, why would he tell Sugar, of all people?
3) Randy develops the strategy of pissing everybody off, and then blindsiding them by playing an idol. - This strategy would make sense ONLY if Randy new that Bob had the idol. But, he had no reason to believe that Bob had the real idol, especially after Sugar spent 5 days on Exile Island, ans some people already knew she had the idol.
4) At the same time that Randy is developing his strategy, Sugar was hatching a nearly identical plan. - Even if Sugar is indeed a bitch, it's a rather elaborate, and random plan for her to come up with on here own.
5) Sugar's strategy, and Randy's strategy happen to jibe PERFECTLY - What a coincidence!
6) Bob, who has been portrayed only as a sweet, and kind person, agrees to humiliate somebody who has never crossed him in any way. - Bob doesn't seem that vindictive, and there's no benefit in him getting closer to Sugar, when she's a pawn with no power.
If it hadn't been for this "twist," it would have been a very boring show, since everybody (players and viewers) knew Randy was going to get the boot. To save the episode, the production team pulled aside all of the relevant contestants, and instructed them on what to say and do. If they showed any reluctance to comply, the producers would just remind them of the contracts they signed, which puts them at the mercy of the Survivor franchise.
